NOW OFFERING TELEMEDICINE!
For some regular business hours as well as some after-hours eye emergencies, having to meet us at our office can be time consuming and avoidable. We may be able to offer you a “virtual consultation” (which could include diagnosis and treatment) from the comfort of your home through our telemedicine program called “EyeCareLive.” Below are seven simple step-by-step instructions on how to access the program.
Once you have registered, you will be able to submit a request for a consultation. We will get back to you at our first opportunity to schedule a convenient time to “meet.” Make sure your device is set to receive notifications so you will be aware when we try to reach you.

Open your smart phone’s camera and point it at the QR code. This will lead you directly to the EyeCareLive application.
You may instead search for “EyeCareLive” In the App Store or Google Play.
Look for the above logo and download the application.
Now it’s time to get started! Please sign up by supplying your full name and email address, then create a password. Please allow EyeCareLive to have access to your camera and microphone so you can appropriately communicate with the doctor.
Update your Personal Information in ”My Account” by clicking on the upper left hand corner menu. You will be required to enter a valid credit card under “Payment Information.” You may be charged between $0-$100 depending on the complexity of the consultation determined by your doctor.
To connect with us, click on the top left corner menu then “Find Doctor.” You can search by each doctor’s name or add in our code AZ3937 to be directly linked with us. From there, you may select either Dr. Cohen or Dr. Goldberg.
After selecting the doctor, you will be taken back to the patient dashboard. Select “Make an Appointment” on the dashboard, and select the appropriate type of consultation.
Now you may begin the consultation! Simply describe your symptoms, record an audio message, and/or send photos and videos directly to the doctor. When finished, select “Next” to schedule the date and time to speak with us, at your convenience.
This is your patient dashboard. You may be asked to complete an Eye Test prior to your virtual consultation, which can be found here. You may also directly message us any time by clicking “Message My Doctor.”
